Data Subject Application Form
Under the Personal Data Protection Law No. 6698 (the “Law”), personal data subjects, as defined as “data subjects,” or their legal representatives are granted the right to submit requests regarding the processing of personal data pursuant to Article 11 of the Law.
Pursuant to Article 11 of the Law, data subjects have the right to:
to learn whether his/her personal data are processed or not,
to demand for information as to if his/her personal data have been processed,
to learn the purpose of the processing of his/her personal data and whether these personal data are used in compliance with the purpose,
to know the third parties to whom his personal data are transferred in country or abroad,
to request the rectification of the incomplete or inaccurate data, if any,
to request the erasure or destruction of his/her personal data under the conditions referred to in Article 7,
to request reporting of the operations carried out pursuant to sub-paragraphs (d) and (e) to third parties to whom his/her personal data have been transferred,
to object to the occurrence of a result against the person himself/herself by analyzing the data processed solely through automated systems,
to claim compensation for the damage arising from the unlawful processing of his/her personal data.
You are required to submit your requests regarding your rights to us in writing or through other methods determined by the Personal Data Protection Board. Requests will be concluded as soon as possible and/or within 30 (thirty) days. However, if fulfilling your request incurs additional costs, a fee may be charged in accordance with the tariff determined by the Personal Data Protection Board.
APPLICATION METHODS
Application by Mail or In Person: You may submit your completed and signed application form/petition, together with documents verifying identity, either in person or by mail to Harbiye Mah. Asker Ocağı Cad. Süzer Plaza No: 6 İç Kapı No: 120 Şişli / İstanbul.
Application via Registered Electronic Mail (KEP): You may send an e-mail from your registered electronic mail (KEP) address to sanus@hs03.kep.tr, including your signed petition or application form;
Application via E-mail Using Mobile Signature or Secure Electronic Signature: You may send an e-mail to info@sanusgroup.com including your petition or application form signed with a mobile signature or secure electronic signature, together with documents verifying your identity;
Application via E-mail Using the E-mail Address Registered in the Data Controller’s System: You may send an e-mail to info@sanusgroup.com from the e-mail address previously notified by you to Sanus Sigorta Brokerliği Anonim Şirketi and registered in Sanus Sigorta Brokerliği Anonim Şirketi’ system, including a completed and signed application form/petition and documents verifying identity;
Application via a notary public or other legally valid methods with identity verification is also admissible.
(If the application is submitted by a proxy, please include a power of attorney evidencing your authority with your application.)

In the event that my personal data are corrected, deleted or destructed upon my application, I request that such correction be notified to the third parties to whom my personal data have been transferred.
I request compensation for the damages I have suffered due to the unlawful processing of my personal data.
